Rhode Island Monthly’s Excellence in Nursing Awards Nomination Form

General instructions
All winners will be selected by an expert panel of nurse leaders from outside of Rhode Island. Please review all of the categories below before determining which to nominate your nurse for, and please be sure to fill out the entire application.

Nominees must meet the following general criteria to be considered:
1. The nominee holds a license to practice nursing in the state of Rhode Island.
2. The nominee has served/practiced in Rhode Island in the area/position for which he/she is nominated in the past year.

AWARDS CATEGORIES:

Nurse of the Year in Academic Setting
This award is given to an outstanding nurse educator involved in a formal entry-level or graduate level nursing education program. This award may also recognize a registered nurse who has led significant formal or informal nursing research which has impacted health care or community.

Advanced Practice Registered Nurse

This award honors the certified registered nurse anesthetist, clinical nurse specialist or nurse practitioner.

Clinical Nurse Educator of the Year
This award honors the registered nurse educator in any health care setting who is involved in ongoing continuing education programs for nursing staff.

Clinical Practice Nurse of the Year
This award recognizes a registered nurse delivering exemplary patient care in the clinic, hospital or office setting.

Community/Home/Public Health Nurse
This nurse works in a setting that renders care within the home, community, or public health sphere.

Licensed Practical Nurse
This award recognizes a practical nurse delivering exemplary patient care in the clinic, hospital or office setting.

Long Term Care Nurse
This nurse works in a skilled nursing care or assisted living setting.

Nurse of the Year in Non-Traditional Setting
This award honors a nurse best exemplifying the art and science of nursing in a non-traditional employment setting (law firm, public policy, health care management, quality assurance, military, school, etc.).

Nurse Entrepreneur of the Year
This award honors a nurse who combines their healthcare knowledge and business sensibilities to develop successful business ventures that center around optimal care delivery.

Nurse Executive
This nurse works within the capacity of a Chief Nurse Officer or Chief Executive Officer of a hospital or health care organization.

Nurse Leader of the Year
This award recognizes a registered nurse who administers the business/patient care activities of a health organization and/or who manages other personnel.

Recent Nurse Graduate
This award recognizes a recent nurse graduate (within the last five years) who exemplifies future leadership skills to enhance the profession.

Nursing Staff/Team of the Year
This award honors a staff, unit or team of nurses in a hospital or health care facility setting who work together to deliver outstanding patient care.
